Geriatric Care Managers are
health and human services professionals with specialized
knowledge and experiences related to aging and elder care
issues. We assist older adults and persons with
disabilities in attaining their maximum functional potential
to reach the best quality of life possible. We are
sensitive to preserving the dignity and respect of each
individual. We are an experienced guide and resource for
families of older adults and others with chronic needs.
Services may include:

Comprehensive care
assessments – identifying health, physical, social,
emotional and financial needs.

Individualized planning
for short and long range solutions

Liaison for long distance
family caregivers.

Continuity of care by
facilitating communication and coordination of services
with health care professionals, family and long distance
caregivers for the benefit of the elder.

Arranging for home care
and home management supports to allow elders to stay in
their own home.

Relocating elders to
alternative housing, assisted living or nursing homes.

Information, education and
referrals on medical, legal, financial matters, and
resources that relate to elders.

Coordinate and monitor
services and service providers.

Crisis Prevention and
Intervention.

Advanced Directive
Planning – healthcare power of attorney, living wills,
etc.
